Representatives of the Robinson Tennis and Golf Academy and or/commercial media may be represented during our practices or tournaments. Occasionally, such groups will interview or photograph students for promotional purposes in the interest of the tennis and golf academy. In order for such images or recordings, a release and authorization is required on behalf of your child. Please review the statement below and sign and date on the appropriate line, and return this form to your child’s tennis instructor. Thank you for your cooperation.

I hereby authorize the Robinson Tennis and Golf Academy, and the commercial media, acting through their authorized employees or agents and in their discretion to use, reuse, publish, republish, and copyright audio or visual reproductions of the above-named student’s voice or image alone, with other persons, and with or without the use of the students name. I am the parent /guardian of the above-named student.
